Our Role
CareBridge provides non-emergency medical transportation and appointment accompaniment services. We do not provide medical care, diagnosis, treatment, or medical advice. Our role is limited to transportation support and, when authorized by the client, relaying information provided by licensed
medical professionals to designated family members or caregivers.
Protected Health Information (PHI)
Protected Health Information (PHI) includes any information that can identify you and relates to your health, medical condition, or healthcare services. CareBridge takes the privacy and security of your PHI seriously.
How We Use and Share Information
CareBridge may use or disclose your information only as necessary to coordinate transportation services, assist with appointment accompaniment, communicate with healthcare providers when authorized, and relay factual information to family members as directed by you. We do not interpret
medical information or provide opinions or advice.
Your Privacy Rights
You have the right to request access to your information, request corrections, limit certain disclosures, and receive a copy of this notice at any time.
Safeguards
CareBridge maintains administrative, physical, and technical safeguards to protect your information in compliance with HIPAA regulations. Client information is never sold or shared for marketing purposes.
